SKIN B-CELL LYMPHOMAS: 20 YEARS EXPERIENCE IN OBSERVATION AND TREATMENT

Abstract

Authors observed 18 patients with cutaneous B-cell lymphomas (B-CLS): primary cutaneous follicle center lymphoma (PCFCL) (44,45%), primary diffuse large B-cell lymphoma (B-CLL) (44,45%), cutaneous marginal zone B-cell lymphoma (CMZL) (11,1%). Features of cutaneous marginal zone B-cell lymphoma are: appearance of multiple spots; identification of diagnosis in stages T2aN0M0; treatment - excision or laser destruction; frequent recurrence do not affect prognosis. Features of primary cutaneous follicle center lymphoma are: appearance of single node; identification of diagnosis in stages T1aN0M0 and T2aN0M0; treatment - excision, can be also combined with radiotherapy. Features of primary diffuse B-CLL are: appearance of multiple spots; identification of diagnosis in stages T3bN0M0, T1aN1M0 and T2bN2M0; disease is aggressive; treatment is a local radiotherapy, interferon alpha intralesional, chemotherapy according to schemes CHOP, COP and R-CHOP, photodynamic therapy is possible.

A CASE OF ACRAL LENTIGINOUS MELANOMA WITH EVALUATION OF С-KIT GENE EXON 11 MUTATION STATUS

Abstract

Acral lentiginous melanoma type is characterized by specific clinical and histologic features therefore it requires a special approach to diagnosis and therapy. c-KIT oncogene mutations are frequent in this tumor type accounting up to 23% of all this type melanoma cases. The presence of exons mutations in c-KIT gene indicates therapy of tyrosine kinase inhibitors. The clinical case of acral lentiginous melanoma with the analysis of the mutational status c-KIT 11th exon that is considered as one of the most significant sites, is presented. The functional role of mentioned substitutes was not described previously although tyrosine kinase inhibitors could be effective for such type melanoma patients. Number of mutations, which role in the activity of c-KIT receptor and therapy response have not been studied, is presented.

INNOVATIONS TO TOPICAL THERAPY OF ROSACEA

Abstract

The main pathogenetic aspects of rosacea, various classifications of dermatosis and the modern clinical classification are presented. The symptoms that differentiate rosacea from other dermatoses are described. Contradictory of modern scientific concepts of different researchers, approaches to etiology, pathogenesis and treatment has been noted. The social significance of rosacea, the importance and possibility of eliminating the symptoms of the disease at early stages, the achievement and prolongation of remission are shown. The main triggers for the manifestation of rosacea, which should be taken into account to achieve the maximum effect in the treatment of the disease, are indicated. The domestic and foreign clinical recommendations on the treatment of the disease are considered in view of the appearance of new external drugs. The results of several studies in the USA and Canada on the efficacy of 1% ivermectin cream and 0.5% brimonidine tartrate gel compared to placebo are shown. The first domestic positive experience of self-study with 1% ivermectin cream (Solantra®) and 0.5% brimonidine tartrate gel (Mirvazo Derm®) is presented. The high efficacy and very good tolerability of this drugs, as well as the absence of any side effects in patients with moderate to severe rosacea are shown.

REDERMALIZATION OF THE SKIN AS A PATHOGENETICALLY VALID METHOD IN THE PERIOD OF CONVALESCENCE OF DERMATOSES

Abstract

The skin repair process with the method of redermalization after atopic dermatitis complicated by streptodermia is described. The analysis of domestic and foreign literature on the validity of redermalization use in the programs of patients recovery after the transferred dermatoses is given. The main mechanisms of appearance of pathological skin changes during the reconvalescence of atopic dermatitis are described and pathogenetic substantiation of the possibility of their correction with the help of redermalization is given. Control of skin changes at the structural level was carried out with confocal scanning laser microscopy (KSLM) to evaluate the efficacy of atopic dermatitis therapy during the reconvalescence with redermalization method.

PSORIASIS VS MALIGNANCIES: POSSIBLE INTERRELATION AND POTENTIAL RISKS

Abstract

To date one of the most discussed problems in the context of psoriasis is the possibility of correlation between the psoriatic process, considering its systemic nature, and the risk of cancer developing. Such interrelation can be caused both by systemic chronic inflammatory process and by methods of therapy. The purpose of this literature review is to assess the risk of developing malignant neoplasms of various organs and systems, as well as the factors that determine it, in patients with psoriasis in comparison with the general population. The mechanisms of action as well as early and remote side effects of the current treatment are described. The literature data on the risk of their carcinogenic effects are presented. The results of studies of a number of other factors acting as possible initiators of carcinogenesis in patients with psoriasis are highlighted, among them: the presence of a systemic inflammatory reaction, the severity and duration of the course of the disease, and also the comorbid states.

THE CONDITION OF THE PSYCHOEMOTIONAL STATUS IN PATIENTS WITH DIFFUSE TELOGEN HAIR LOSS

Abstract

Diffuse telogenetic alopecia is a polyethological disease accompanied by a disruption in the operation of the hair follicles cells and leading, in the future, to the growth cycle failure and development of the hair, which subsequently leads to thinning and excessive hair loss. The results of the study of psychoemotional disorders in patients with telogen hair loss identified by integrated analysis of anamnestic data, patient’s complaints, the survey using validated psychotherapeutic tests and subsequent consultation with a doctor-psychotherapist are presented. The relationship between personality type, psychological state and the severity of clinical manifestations was detected. The dominant triggering factors of development of diffuse alopecia severe forms were determined. The relationship between the severity of clinical manifestations and the position was found. Based on the analysis of the obtained data, literature review and the example of two clinical cases of patients with diffuse telogenous alopecia was shown the importance of identifying correction of alexithymia and depression with the aim of increasing the effectiveness of therapy.

RESULTS OF OXYGEN-OZONE MIX IN THE TREATMENT OF VITILIGO

Abstract

Obtaining new data on the role of the antioxidant system in vitiligo in the future can contribute to the development of new effective and safe methods for combining vitiligo with an oxygen-ozone mixture with a minimal risk of side effects, as well as a short duration of treatment and low financial costs. Materials and methods. We observed 50 patients with vitiligo in the advanced stage of the disease, aged 20-59 years. Before the start of the course of ozonotherapy and after its completion in 2-4 weeks, the main indicators of the antioxidant system were studied (the following superoxide dismutase, glutathione peroxidase, glutathione, malonic dialdehyde, 8-OH-deoxyguanosine). All patients received systemic oxygen-ozone mixture therapy by intravenous injection of ozonized saline (OBS). The amount of the injected solution is 300-400 ml, the duration of the procedure is 30-60 minutes. The procedures performed 3 times a week; the course is 9-10 procedures. The tolerability of the procedures was satisfactory in all cases, no side effects were observed. Results. As a result of the treatment the stop of the progression of vitiligo was noted in 40 (80%) patients in the form of diffuse or point repigmentation, a decrease in the area of depigmented spots. This clinical picture correlated with the normalization of the indices of oxidative stress, which was manifested by a significant decrease in the level of malonic dialdehyde and 8-OH-deoxyguanosine against a background of an increase in the concentration of glutathione and glutathione peroxidase on average in 1.5 times.

CHRONIC ACTINIC DERMATITIS AND PATHOLOGY OF THE DIGESTIVE TRACT

Abstract

Chronic actinic dermatitis (CAD) is characterized by itchy eczematous lesions on areas of skin exposed to sunlight. The disease affects men middle-aged and elderly. Three criteria was taken into account to diagnose CAD: the effects of the minimum erythemal doses of UVA and UVB, a persistent eczematous lesions in areas exposed to radiation and sometimes non-irradiated sites, and histopathological changes resembling chronic eczema. The eczematous process of the skin develops as a result of a number of factors that lead to sensitization of the organism, including in the pathology of the digestive tract and the hepatobiliary system. A correlation was found between the severity of eczematous skin lesions and the severity of the digestive tract disease. Material and methods. Under the care of the Department of dermatology and venereology Clinics of the Samara state medical University of Minzdrav of Russia were 65 men with CAD, among them 61 (93.9%) patients are over 40 years old. The pathology of the digestive tract and the hepatobiliary system was detected in 51 (78.6%) patients. Concomitant diseases of the digestive tract and the hepatobiliary system had a latent period and were identified for the first time due to a comprehensive survey in 37 (57%) patients. Results. In most cases, CAD on the background of the digestive system disease, there was a moderate and severe course of the eczematous process. The frequency of occurrence of reflux esophagitis, superficial and atrophic gastritis, duodenogastric reflux, as well as the combined pathology of the hepatobiliary system was maximal in patients with the most severe course of the eczematous process. The majority of patients without pathology of the digestive tract and hepatobiliary system had minimal skin manifestations of eczema.

CLINICAL EVALUATION OF THE EFFECTIVENESS OF RADIOFREQUENCY FRACTIONAL ACTION IN INVOLUTIONAL SKIN CHANGES

Abstract

The results of the study of biorevitalization effectiveness of the facial skin in women aged 30 to 40 years are presented. Material and methods. In the 1st group (n = 30) hyaluronic acid (HA) was used along the face oval, in the zone of the cheekbone, along the nasolabial folds, along the longitudinal wrinkles in the forehead area with the use of “micro-puncture” and “short-linear” techniques in the medium-deep layers of the dermis. The distance between injections is 1 cm. Injection schemes are standard, proposed by manufacturing companies - once a week, in total 6 procedures. In group 2 (n = 30), the HA was administered with the Legato apparatus, where two technologies were successively used: radio frequency and ultrasound, once every 2 weeks, with a course of 5-6 procedures. Both groups used HA in a concentration of 2 mg/ml Gialripyer-07M/mesolift. Instrumental studies of the functional properties of the skin included assessment of hydration of the epidermis (corneometry), deformation and elastic properties of the skin (cutometry), assessment of transepidermal loss of moisture (TEVA-metry), assessment of seboregulatory function of the epidermis (sebometry) using Multi Skin Test Center® MC750; dermatoscopy on a digital video camera Aramo SG, providing a computer image of the texture of the skin and its moisture level; ultrasound scanning of the skin using a digital high-definition system DUB (“TPM GmbH”, Germany) and a high-frequency linear 17 MHz sensor in SmParts Superfic scanning mode with a resolution of 78 microns. Results. The study shows the superficial effect of mesotherapy procedures: improving the morphofunctional properties of the skin, they have no effect on the thickness, echogenicity and relief of the dermis. At the same time, the obtained results of instrumental studies of the structural and functional parameters of the skin make it possible to justify the use of modern technology Legato (radio frequency and ultrasound) for intradermal administration of HA preparations, with the help of which it is possible to obtain in a short time a positive dynamics of the indices of structural and functional parameters of not only surface layers of skin, but also at the level of the dermis.

STUDY OF THE MESOTHERAPY EFFECT WITH EPIPHYSIS PEPTIDES ON THE MOISTURE AND ELASTICITY OF THE SKIN

Abstract

Introduction. Mesotherapy is a widely used method of preventing external signs of aging in cosmetology. However, drugs that have proven effectiveness are not enough, therefore epiphyseal peptides are of great interest. Purpose: is to study the effect of intradermal administration of peptide epiphysis on the functional para-meters of the skin. Material and methods. There were 38 women in the research devided into 2 comparable groups. The main group (26 persons) was treated by mesotherapy with epiphyseal peptides. Face, neck and decollete zones were treated by the method of “deep nappage” once a week, 5 procedures per course. Isotonic solution of NaCl was similarly injected into the neck and decollete areas of the control group (12 people). The moisture content (Corneometer CM 825, “Courage & Khazaka”, Germany) and the elasticity (Cutometer MPA 580, “Courage & Khazaka”, Germany) of the skin were determined. Functional skin parameters were evaluated before and after treatment (in both groups) and also on the 1st and 6th months after mesotherapy in the main group. Results. Under the influence of epiphysis peptides the moisture content of the skin increases by 10% after treatment (in the control group without changes) and after 1 month, then decreases by 5% in 6 months below the initial values. In the main group R0 and R8 parameters increase immediately after treatment by 21% and 20% (in the control group R0 and R8 decrease by 4-6%), then decrease by 6-7% and by 28%. The R4 parameter increases immediately after treatment by 20-25% (in the control group it decreases by 13%), then decreases by 12% and 24%; F0 and F1 parameteres increase immediately after treatment by 17% and 22% (in the control group they decrease by 4% and 3%), then decrease by 11% and 9%, by 25% and 28%. Conclusion. The revealed dynamics of skin functional indexes may support the stimulating effect of peptide epiphysis on the cellular elements of the dermis (activation of metabolic processes after treatment and improvement of elasticity parameters by 25-28% after 6 months). Mesotherapy with epiphyseal peptides can be recommended as a method of prevention and correction of age-related skin changes.

CONFOCAL SCANNING LASER MICROSCOPY IN ASSESSING THE EFFICACY OF AUTOLOGOUS PLATELET-ENRICHED PLASMA WHEN CORRECTING AGE-RELATED CHANGES IN FACIAL SKIN

Abstract

The efficacy of using autologous platelet-enriched plasma (auto-OTP) in women of different age groups with involutionally altered facial skin by the method of confocal laser scanning microscopy was evaluated. Own results of treatment of 50 patients aged from 35 to 55 years with clinical manifestations of involutionally altered facial skin are presented. As a result of the therapy, a decrease in the clinical signs of aging was observed in all women, but the clinical effect was more pronounced in the age group from 35 to 45 years (97%, of which 76% was significant improvement, 21% was improvement). In the second group (46-55 years), the decrease in clinical signs of aging after the course of procedures was observed in 86%, of which a significant improvement was in 39%, improvement in 47%, without effect 14%. With the help of confocal laser scanning microscopy it was established that in all patients the intradermal administration of auto-OTP promoted the improvement of the structural and functional organization of the epidermis and dermis, increasing the thickness of the dermal layer. The received data testify to high efficiency and safety of application of auto-OTP in correction of involutional skin changes.

PAGES OF HISTORY OF THE KAZAN SCIENTIFIC SCHOOL OF DERMATOVENEREOLOGY: THE LIFE AND WORK OF V.F. BURGSDORF

Abstract

The main stages of life and activity of V.F. Burgsdorf (1864-1935), his scientific achievements and priorities as the founder of one of the largest scientific schools in Russia are described. The representatives of this school made a huge contribution to the development of national dermatovenereology. Special attention was paid to the contribution of V.F. Burgsdorf in the creation of the second Russia svetlichnoe (phototherapy) offices in the STI clinic (Skin and Venereologic clinic) of A.G. Gе in Kazan on the territory of which currently is the Republican dermatovenerologic Dispensary. The work of A.G. Gе (1889-1925) in the STI clinic became one of the most productive periods of life and activities of V.F. Burgsdorf. In November 1900 V.F. Burgsdorf opens svetlichnoe (phototherapy) office with the support of prof. A.G. GE. Due to his medical and public authority, V.F. Borgsdorf received a large donation from the Empress Maria Feodorovna for his further development. V.F. Burgsdorf devoted much of his life to the development of treatment and prevention of skin and venereal diseases, organization of scientific researches and improvement of medical education in Russia. V.F. Burgsdorf greatly improved the system of medical care for patients.
