Metatypical basal cell carcinoma in young man

Abstract

Malignant epithelial skin tumors are leading among oncological diseases worldwide. In most cases epithelial skin tumors affect areas, susceptible to the action of adverse environmental factors. Metatypical basal cell carcinoma is a variant of basal cell carcinoma that combines the features of basal cell and squamous cell carcinomas. It characterized by infiltrative growth and the destruction of the underlying tissues, pain, bleeding, formation of distant metastases and frequent relapses. Metatypical basal cell carcinoma is more common in women, with the average age is 70.5 years. Historical data, the results of own clinical investigations, rare case of metatypical basal cell carcinoma of the young man, who was successfully treated with recombinant interferon-a2 and prospidinum are presented.

Cutaneous toxic effects of targeted therapy: clinical manifestations and correction

Abstract

Targeted therapy used in oncology today is the most effective, but frequent cutaneous side effects force to reduce dose or interrupt treatment, significantly reducing the anti-tumor effect. The data on the pathogenesis of toxic skin reactions and the most frequent clinical manifestations (papular-pustular reaction, hand-foot syndrome) according to various studies are presented. The basic principles of correction are described. Examples of anticancer drug doses correlation and severity of skin reactions are presented.

Systemic lupus erytherymatosus: an interdisciplinary approach to the diagnosis

Abstract

Diagnosis of systemic lupus erythematosus presents certain difficulties up to now. Due to the prognostic significance of the disease and the need to early pathogenetic treatment clinical cases of SLE are described. Questions of modern differential diagnosis are reviewed. If skin changes are presented a positive test for antinuclear antibodies helps to detect SLE timely. 4 clinical cases of women with systemic lupus erythematosus are presented. Diagnostics required comprehensive examination with histopathological and immunological studies: blood test antinuclear factor on HEp-2 cells with the definition of lupus anticoagulant in blood plasma, rheumatoid factor, anticardiolipin antibody IgG and IgM, double stranded DNA, antinuclear antibody IgG, auto antibodies to IgG AMA-M2 (immunoblott), cardiolipin antibodies and lupus anticoagulant.

Trace element status of the patients with alopecia areata

Abstract

The issue of trace elements participation in the pathogenesis of alopecia areata and the mechanisms of change in bio-concentrated trace remains unresolved and requires further consideration. Goal. To rate trace element balance in patients with alopecia areata. Material and methods. The study group consisted of patients with focal forms of alopecia areata (n = 100), the comparison group - healthy individuals (n = 100). The estimate of trace element status was performed by comparative analysis of the contents 11 trace elements (zinc, iron, copper, cobalt, chromium, manganese, nickel, strontium, bismuth, cadmium, lead) in whole blood and hair in the study group. Results. The study reported a significant decrease in the level of zinc and iron in a patient’s blood and an increase in bio-concentrate iron, copper, chromium and nickel in hair ofpatients compared with healthy participants. Trace element analysis of whole blood and hair allowed to suggeste that a cause of an arising microelement imbalance is probably an autoimmune inflammation in the skin ofpatients suffering from alopecia areata. Confirmation of this hypothesis requires further comprehensive studies.

Expression of heat shock proteins HSP90 on neutrophils peripheral blood in patients with atopic dermatitis

Abstract

HSP90 expression on the neutrophil plasma membrane has been investigated in patients with atopic dermatitis. Methods. Human peripheral blood cells were stained with monoclonal antibodies to the HSP90 and were analyzed using the flow cytometer. Results. Neutrophils obtained from patients with atopic dermatitis have been found to express the more significant level of HSP90 on plasma membrane, and percentage of these cells has been shown to be higherfor atopic patients compared with healthy donors. Both, neutrophil count and HSP90 fluorescence were increased according elevation of index SCORAD for atopic patients. The most significant elevation in percentage of neutrophils expressing HSP90 on plasma membrane has been found during the first and second weeks of acute phase of the disease. We did not found the significant differences for HSP90 expression on neutrophils dependent on IgE level in the blood serum. Conclusion. HSP90 expression is elevated on the neutrophil plasma membrane from atopic dermatitis patients compared with healthy donors. Percentage of these cells were positive correlated with SCORAD index.

Hidradenitis suppurativa: clinic and diagnostics (part 2)

Abstract

The data on the clinical features and course of suppurative hidradenitis characterized by severe course and a tendency to relapse are presented. To select the treatment strategy the scale of the severity and staging process are proposed. The atypical localization of purulent suppurative hidradenitis and follicular occlusion tetrad, which also includes conglobata acne, folliculitis sycosiformis atrophicans / exfoliating scalp cellulitis and pilonidal abscess are described. It is proved that manifestation of suppurative hidradenitis are phenotypically heterogeneous. The role of genetic, comorbid factors, metabolic and hormonal disorders, as well as the role of the immune system in the development of the disease are discused. For the diagnosis index Sartorius (Sartorius score), microbiology, immunohistochemistry and other research methods, which largely determine the tactics of treatment of severe dermatosis were used.

The analysis of literature and exceptional clinical case of combination of inverse acne and infiltrative-productive stage of rosacea are presented. The historical facts, ethiopathogenesis, comorbidity, treatment, clinical picture and diagnostics with use of both standard and invasive methods of research (histological method) are described. Successful treatment of combination of inverse acne and an infiltrative-productive stage of rosacea with antibiotics and actinolyzat is reported.

Vitiligo treatment algorithm

Abstract

The treatment strategy ofpatients with vitiligo, including the identification of motivation for long-term treatment and informing patients to avoid false expectations as causes of dissatisfaction and early termination of the therapeutic course are described. The algorithm for the treatment of vitiligo consists of five successive stages. Each of them is sequentially applied with no previous treatment regimens efficiency or its maximum possible effect achievement. The use of the described treatment algorithm allows us to achieve in 70-80% of cases a marked repigmentation of vitiligo and to ensure long-term and often life-long remission of the disease.

Topical treatment of chronic dermatoses with kertoplastic agents

Abstract

Atopic dermatitis and psoriasis are common chronic dermatosis characterized by prolonged recurrent course. Depending on the severity of the disease topical and systemic treatment are applied. Keratoplastic agents play an important role in treatment of skin diseases. These agents include Naftalan Cream 30%, which significantly improves the skin condition. The results of treatment of 10 patients with atopic dermatitis and 10 patients with psoriasis vulgaris Naftalan cream 30% are presented.

Melanocortin 1 receptor MC1R as a genetic factor in the development of psoriasis

Abstract

The CRH-POMC system in the skin coordinates pigmentation and the immune response. We performed the association study of single-nucleotide polymorphism SNPs of MC1R gene in a cohort ofpsoriasis patients of Tatar ethnic group from the Volga-Ural region of Russia. The results suggest that polymorphisms of the MC1R gene may contribute to the protection or development of psoriasis in Tatar population.

Infantile hemangioma: current classification, clinical picture and effective methods of therapy

Abstract

Review of the literature devoted to one of the most common tumors of childhood - hemangioma is presented. Literature search was performed using the NCBI, including PubMed, PubMed Health, and The Cochrane Library databases. Infantile hemangioma is a neoplastic proliferation of endothelial cells, which is characterized by different phases of development: proliferation and spontaneous involution. The types of infantile hemangiomas, variants of the course and possible complications such as ulceration, functional disorders, and the formation of cosmetic defects are described. Incidence associated with infantile hemangioma syndrome is presented. The current views on the pathogenesis of infantile hemangiomas are described. Information about the best methods of diagnostic imaging, as well as medical and surgical therapies, including systemic and topical therapy with β-blockers, corticosteroids and use of lasers are described.

Biologically false positive serologic reactions for syphilis in patients with benign uterus tumors

Abstract

The mechanism of formation of active and chronic false positive reactions serologic syphilis tests is described. Spontaneous negativation of acute false positive serologic reactions develops in 6 months. Chronic false positive serologic reactions persist for a long time, sometimes during the whole life time. Chronic false positive serologic reactions for syphilis are the diagnostic criteria for systemic lupus. Chronic false positive serologic reactions before and during the evaluation of clinical symptoms in other connective tissue diseases, endocrinologic, oncologic pathology are described. The Hughes ’syndrome is the most often cause of false positive serologic syphilis tests. 3 cases of false positive serologic syphilis tests in patients with benign uterus tumors are presented.
