EXPRESSION OF NUCLEAR MATRIX METALLOPROTEINASE-2 IN PATIENTS WITH CUTANEOUS MELANOMA DEPENDING ON THE AVAILABILITY OF MUTATIONS IN THE PPP6C GENES

Abstract

Material and metods. Tumor samples from paraffin blocks of melanoma patients obtained from Krasnoyarsk Territorial Oncologic Center were used in the study. Skin tissue samples (n = 40) were received from the Krasnoyarsk Territorial Pathological Anatomy Bureau. The mutation in the exon 7 of the PPP6C gene was determined by the Sanger sequencing method using specific primers. Immunohistochemistry was performed based on a standard technique using primary anti-matrix metalloproteinase-2 antibodies. Results. Mutations in the gene PPP6C were detected in 12.5% of patients; in all patients with mutation, missense mutations were observed: G266R, I271N, P259H, and a combination of two mutations in the hot spots of the P259L and R264L gene were also determined. It was found that the expression of MMP-2 in patients with a PPP6C mutant tumor type was not similar to a wild type tumor patients (p = 0.72). The predominant expression of nuclear MMP-2 is observed in PPP6C-negative patients, while in patients with mutations in the PPP6C gene, the cytoplasmic expression of MMP-2 was predominant (p = 0.045). We also found a predominance of nuclear MMP-2 expression, as a prognostically unfavorable factor, for patients in the PPP6C of negative group. Both of these facts may be related to the fact that this mutation does not affect other signaling mechanisms not associated with the BRAF and NRAS genes, nor does it directly affect the expression pattern of MMP-2.

ULTRASTRUCTURAL FEATURES OF THE MAIN CLINICAL VARIANTS OF BASAL CELL CARCINOMA, AS A FACTOR DETERMINING THE BIOLOGICAL BEHAVIOR OF THE TUMOR

Abstract

Histological variants of basal cell carcinoma (BСС) are characterized by a variety of structures depending on the location of the original tumor cells, their differentiation, the reaction of the surrounding tissue, the age of the patient, which causes a large number of histological classifications of the tumor. To evaluate the prognosis of the effectiveness of different treatment methods and the biological behavior of the ВCC, the clinical form and the corresponding histomorphological type of tumor are of great importance. Most of the primary foci of the ВCC are surface and micronodular (syn.: nodular) forms, rarely there is a scleroderm-like variant and extremely rarely fibroepithelioma of Pincus. Surface and micronodular forms of ВCC corresponding to the T1N0M0 characteristic (up to 2 cm in diameter) with a simple histological type of structure (multicentric, solidly compact) are regarded as tumors with a low risk of recurrence and progression. Materials and methods. Studying the features of the pathomorphological characteristics of the tumor and the frequency of various variants on the basis of an analysis of 223 ВCC biopsy specimens, we found that most often (54.7%) there was a solid type of tumor, less often -- superficial (12.7%), morphea (4.9% ), adenoid (10.9%), as well as ВCC with a piloidal (3.1%) and sebaceous (4.4%) differentiation. Among the biopsies analyzed in 6.8% of cases metatypic cancer was found. Conclusions: The histological structure of the tumor can determine its biological behavior and have prognostic significance. Thus, pronounced primitive angiogenesis, infiltrating growth and active stroma formation testify to tumor aggressiveness and clinically most correspond to the recurring macronodular and infiltrative clinical forms of BCC. The tumor is most aggressive in its basosquamous transformation, manifested in the emergence of two components of the tumor complex in the area of dyskeratosis and pseudoepitheliomatous hyperplasia of the epidermis: basal cell carcinoma and highly differentiated cancer, which corresponds to the formation of metatype skin cancer.

SUPERNUMERARY NIPPLE OR PAPPILOMA?

Abstract

Supernumerary nipples can be found in approximately 1 % of women. During the second and the third months of embryonic development when the regression of ectodermal thickening of mammary line takes place, incomplete regression with remaining of some foci may occur. These foci can further develop into a supernumerary breast or a supernumerary nipple. In this article, we would like to present five cases of supernumerary nipples. In 80% of cases, there was a single unilateral supernumerary nipple located along the right mammary line. In all the described cases this minor defect was efficiently excised for cosmetic purposes.

MICROELEMENTAL COMPOSITION OF HAIR IN DIFFUSE TELOGENOUS ALOPECIA

Abstract

Adverse environmental conditions may affect the condition of the hair follicle, which require the optimal amount and ratio of minerals. The description of the diagnosis and treatment of microelements in patients suffering from diffuse telogen alopecia is presented. Based on the analysis of data from the literature review and the results of treatment of groups of patients the importance of identifying microelement imbalance in order to apply effective treatments is shown. The results obtained, showing the imbalance of macro - and micronutrients, including essential, are the basis for recommendations for patients with diffuse alopecia additional examination for definition of microelement composition of hair. This allows to prescribe different composition of microelement complexes that ensure the replenishment of certain microelements deficiency. And this provides an opportunity to reasonably resolve the issue of the need to include in the treatment plan detoxification measures to reduce the content of toxic and conditionally toxic microelements in the body.

CASES OF ANETODERMA IN PRACTICE OF DERMATOLOGISTS

Abstract

Clinical observations of atrophic skin of a child at the age of 1 year 7 months with Bleggad-Haxsthausen syndrome, as well as three forms of an anetoderma (Jadassona-Tibierga, Schweninger-Buzzi and Pelizzari) are presented. All patients are male at the age of 14, 16 and 18 years. The 14-year-old boy has the disease during seven years, while the duration of the other patients is two and one year, respectively. Patient with Schweninger-Buzzi anetoderma received treatment in a hospital without visible improvement. The literature review also noted that the disease can develop both primary and secondary. There are three stages of development of Jadasso--Tibiere anderodermia, the first of which is represented by the erythematous spot. However, inflammatory changes may be absent, but for other forms they are generally uncharacteristic: atrophy develops immediately, then wrinkled skin is protruding.

THE RELATION BETWEEN THE CONTENT OF PERIPHERAL BLOOD NEUTROPHILS AND THE BLOOD RELEASED FROM THE SURFACE OF PSORIATIC PAPULA

Abstract

The functional capabilities of neutrophils go beyond the traditional understanding, as phagocytic cells. There are evidences that show the role of neutrophils in antigen presentation to monocytes and lymphocytes, as well as the ability to regulate functions of immune cells through cytokine production. The role of neutrophils in the pathogenesis of psoriasis is not entirely clear, in this connection, the aim of our study was to examine the content ratio of peripheral blood neutrophils and the blood released from the surface of psoriatic papules with the level of expression of adhesion molecules. Material and methods. Clinical and immunological examination of 82 patients with vulgar and exudative psoriasis in the progressing and stationary stage (39 women and 43 men) aged from 20 to 60 years was carried out. Duration of the disease was from 3 months to 10 years. The control group included 50 healthy individuals (28 women and 22 men) aged from 20 to 60 years. They had no chronic diseases or aggravation. The number of neutrophils was determined on the smear of peripheral blood, as well as blood obtained from the surface of psoriatic papule, defined as a phenomenon of spot bleeding of the psoriatic triad. The content of L-selectin (CD62L, FITC), LFA-1 (CD11a, FITC), LFA-3 (CD58, FITC), ICAM-1 (CD54, FITC), L-selectin was measured on a flow cytometer FC-500 from Beckman Coulter (USA) PECAM-1 (CD31, FITC) on neutrophils of venous blood. Results. In venous blood of patients with psoriasis, the number of 2- and 3-segment forms of neutrophils increased statistically significantly in comparison with the control group, but the concentration of neutrophils with 4 or more segments in the nucleus decreased. In the psoriatic papule, the concentration of neutrophilic leukocytes was significantly reduced in comparison with peripheral blood, while in the segmentogram the predominant 2-segment forms of neutrophils were found. In venous blood, in psoriasis patients a statistically significant decrease of neutrophils with a PECAM-1 molecule was revealed, with which the final phase of transmigration into the tissue is connected. Discussion. Thus, in patients with psoriasis a statistically significant increase in the number of 2-segmetrix neutrophil forms indicates their stability in the lesion

OVERVIEW OF MODERN STUDIES OF COMORBIDITY IN PSORIASIS

Abstract

Psoriasis is a multifactorial inflammatory skin disease associated with various medical conditions. The article presents an overview associations between psoriasis and ulcerative colitis, diabetes, chronic neuropathy, depressions, lymphomas, malignant processes, especially lymphoproliferation, metabolic syndrome. Patients with psoriasis, particularly if disease is severe, are at increased risk of cardiovascular diseases, such as hypertension, myocardial infarction, and stroke. Have examined the association between psoriasis and glomerulonephritis as well as chronic kidney disease. Factors influencing the course of psoriasis include age, stress, smoking, alcohol, a violation of diet, the presence of chronic obstructive pulmonary disease, sleep apnea, mental disorders The article presents an overview associations between psoriasis and metabolic syndrome. When a subject has three of the five listed criteria, a diagnosis of the metabolic syndrome can be made. The criteria listed include glucose intolerance presenting higher fasting glucose 6,1; increased waist circumference or abdominal obesity (≥ 102 cm for men and ≥ 88 cm for women); raised triglyceride levels ≥ 1.7 mmol/l; reduced high-density lipoprotein < 1.04 mmol/l for men and < 1.30 mmo/l for women; and elevated blood pressure 130/85 mmHg. In patients with psoriasis found a significantly increased prevalence and severity of coronary artery calcification.

TREATMENT OF POSTOPERATIVE SCARS WITH COMBINED USE OF CO2 LASER ABLATION AND AUTOLOGOUS PLATELET ENRICHED PLASMA

Abstract

In modern dermatology and cosmetology, the task of developing a complex method of therapy for the symptomatic complex remains acute, despite a large number of studies on this problem. The effectiveness of the treatment of postoperative scars was evaluated by the combined use of laser ablation (ablative photothermolysis) on the SmartXide Dot (Deka) device and autologous platelet enriched plasma (auto-OTP). The result of treatment of a patient with clinical manifestations of atrophic and hypertrophic postacne scarring 4th grade (according to the scorecard scale of post acne D. Goodmantal, 2006) on the skin of the face and back is presented. The tactics of managing the patient during rehabilitation after combined use of laser ablation and auto-OTP with application of topical retinoids are described. As a result of the therapy, there were fewer clinical manifestations of atrophic scars on the face skin on 70-80% and mainly hypertrophic scars on the skin of the back on 50-60%.

Abstract

According statistics, fillers occupy a leading position among non-surgical procedures. Despite their diversity in Russia, the evidence base for the treatment of age-related changes in the skin according to number of clinical and instrumental methods in the diagnostic is not enough. The combination of hyaluronic acid (HA) injections with fillers is pathogenetically justified. Therefore the search for scientifically based protocols for the correction of skin ageing is an actual trend in cosmetology and dermatology. Purpose. To assess the safety and effectiveness of correction of involuntary changes in face and neck skin with drugs based on stabilized hyaluronic acid. Material and methods. 35 women aged 30-65 years were enrolled into two groups by simple randomization. Group I received monotherapy with HA fillers, patients of group II - complex rejuvenation therapy for face and neck: fillers and course of vitalization stabilized HA. The efficasy was assessed according to the Global Aesthetic Improvement Scale (GAIS) on the 1st(M01), 2nd (M02), 3rd (M3), 6th (M06) month of complex therapy instrumental methods (cutometry, corneometry). Safety was assessed during 12 months of follow-up and ultrasound examination of the facial soft tissues. Results. The safety and efficacy injections of stabilized HA have been clinically confirmed. MARKERS OF DAMAGE TO NERVOUS TISSUE IN LIQUOR PATIENTS WITH VARIOUS FORMS OF NEUROSYPHILIS

Abstract

The purpose: to study the features of markers of lesion of nervous tissue and cytokines in patients with various forms of neurosyphilis. Material and methods. The complex clinical examination of patients with syphilis was carried out in the venereological department of the Clinical dermatovenerologic dispensary in Omsk in the period from 2015 to 2017, in order to identify signs of damage to the nervous system. Results: the presence of regularities and features in the deviation of the immune status in neurosyphilis was found. Conclusion. Analysis of the immunological changes in the cerebrospinal fluid showed that the level of interleukin 12p40 is significantly higher in the cerebrospinal fluid of patients with neurosyphilis, while the maximum values of these indicators are in patients with manifest forms of pathology. In patients with neurosyphilis, the concentration of glial fibrillar acidic protein in CSF was statistically significantly increased then in comparison group.
