ITCHING IS A MANIFESTATION OF THE PARANEOPLASTIC SYNDROME

EFFECTIVE ROFERONE A THERAPY FOR THE TUMOR STAGE OF MYCOSIS FUNGOIDES: A CLINICAL CASE

Abstract

A 34-year old female patient with the tumor stage of mycosis fungoides was treated (monotherapy) with roferone A (Switzerland). The treatment was carried out during 1 year. Complete remission lasting for more than 32 months was attained. This significant clinical effect as a result of interferon monotherapy without chemotherapeutic drugs and corticosteroids was attained for the first time in Russia.

STEVENS—JOHNSON’S SYNDROME

Abstract

Stevens—Johnson ’s syndrome (SJS) is a severe total systems allergic delayed (immunocomplex) reaction with skin involvement paralleled by mucosal involvement in more than two organs. The disease incidence increased over the recent decades. A patient with SJS is described with 70% skin involvement. Published data (13 publications) on the etiology and pathogenesis of the syndrome are presented.

SOME CHARACTERISTICS OF PORPHYRIA CUTANEA TARDA IN WOMEN

Abstract

Clinical statistical parameters and laboratory values over the clinical course of porphyria cutanea tarda (PCT), the dermatological syndrome, and the causative factors promoting the disease manifestation in women in comparison with men are analyzed. The results of examinations of 87 patients with PCT (44 women and 43 men) are analyzed. The PCT dermatological syndrome is characterized by similar symptoms in males and females. The characteristic features of the PCT dermatological syndrome in women are more prevalent location of the foci on the skin — upper surface of the feet, forearms, arms, shins. The lesions often look as ecthymas or trophic ulcers. The PCT in women manifests at an older age. Atypical clinical forms of PCT (scleroderma-like, ulcerative necrotic) are recorded only in women. The predominant risk factors are alcohol abuse and HCV infection; in women these factors are 2.5 and 2 times less frequent than in men. Concomitant diseases are polyorgan in males and females; however, chronic hepatitis and gastroduodenitis are 2-fold more incident and gastroduodenal ulcer 5-fold more incident in male vs female patients.

EFFICIENCY AND SAFETY OF ISIDA COSMETIC AGENTS IN THE TREATMENT OF CHILDREN WITH ATOPIC DERMATITIS

Abstract

Atopic dermatitis is one of the prevalent skin diseases. Drugs for external therapy of chronic dermatosis in children should allow free transport of active substances to the cell-cell space and inside the cells, should spare the lipid barrier structure, restore the epidermal barrier and be incorporate in it, filling the damaged sites. Isida, a series of modern liposomal cosmetic means with a DAS (Dorogov ’s antiseptic stimulant), is characterized by these activities. The efficiency of Isida external agents in children with atopic dermatitis is demonstrated. The treatment efficiency was evaluated by the time course of SCORAD (Scoring of Atopic Dermatitis) index and the children ’s quality of life dermatological index.

THE ROLE OF CD45RA+, CD45RO+ LYMPHOCYTES IN THE PATHOGENESIS OF PSORIASIS

Abstract

Psoriasis is a chronic immunity-dependent dermatosis. The formation of T-cellular immune response, leading to the development of autoimmune inflammation, plays the key role in its pathogenesis. Disorders in immunological memory are essential for psoriasis development. We studied the counts ofCD45RA+, CD45RO+ cell subpopulations and the proliferative activity of skin cells in patients with psoriasis during different periods of the disease. The objects of our research were biopsy specimens collected from 41 psoriasis patients during disease progress, 18 psoriasis patients with remission, and from 16 normal subjects. Immu-nohistochemical study of the skin with anti-CD45RA, anti-CD45RO, anti-Bcl-2, anti-Bcl-6, anti-Ki-67, and anti-CD3ε — Ki-67 antibodies. The counts of CD45RO+ cells in the skin of psoriasis patients during the progressive stage were 3.7 times higher than during remission and 14 times higher than in the skin of normal subjects. High expression of Ki-67 protein in the dermal infiltration cells in psoriatic papules was detected. About 30% dermal proliferating cells ofpsoriasis patients during the progressive stage were T-lymphocytes. The counts of Bcl-2+ and Bcl-6+ cells were many-fold increased in psoriasis patients during the progressive stage and remission in comparison with the counts in normal subjects. These data indicate the involvement of T-cell memory in the pathogenesis of psoriasis and the probable proliferation of T-lymphocytes in the skin of patients during psoriasis exacerbation.

ACUTE PHASE PROTEINS IN THE SERA OF PSORIASIS PATIENTS

Abstract

Serum levels of α-macroglobulin (α-MG), albumin, lactoferrin, α-antitrypsin (α-AT), and haptoglobin were measured in patients with slight and severe psoriasis before and after therapy. The concentrations of α-MG and haptoglobin were reduced significantly in severe psoriasis forms. In slight disease the level of α-MG virtually did not differ from that in severe psoriasis, while haptoglobin just tended to decrease. By contrast, lactoferrin and albumin concentrations were significantly higher in slight and severe psoriasis, while α-AT just tended to increase. Correlations of α-MG and lactoferrin levels were detected. The findings confirm the involvement of the studied acute phase proteins in the pathogenesis of psoriasis and recommend their measurements as an additional prognostic test in this disease.

Abstract

Based on analysis of the known approaches to therapy of herpesvirus infections and his own experience, the author suggests modifications in standard therapies for herpes simplex and herpes zoster. Validation of combined ethiopathogenetic therapy of herpesvirus infections is presented, based on the practical and pathogenetic approaches. The author’s supplement to the recommendations of the Russian Society of Dermato-Venereologists is expected to improve the treatment efficiency and arrest the clinical manifestations of infections at any stage of exacerbation.

THE PROBLEM OF SCABIES IN MENTAL ASYLUMS

Abstract

Patients of 4 mental asylums of the Moscow region were screened for scabies. The disease was diagnosed in 56 (14,1%) of 398 patients. Foci of scabies were detected in the majority (60,9%) of inspected wards. Irradiating foci predominated (64,3%). The highest level (20—71%) of scabies involvement was found in supervised wards. The parasitic index in these foci varied from 19 to 80. Specific features of scabies course in mental patients were described. The complex of therapeutic and preventive measures was suggested. The patients with scabies were treated by medifox. The same drug was used for preventive treatment of subjects contacting with the patients. No relapses were recorded.

A CASE WITH SYPHILITIC RESUPERINFECTION IN A HIV-INFECTED PATIENT

Abstract

Syphilitic resuperinfection in a HIV-infected female patient is described. Within 6 months after full-value therapy for early latent syphilis in the presence of reducing titers of serological tests the patient developed a new infection with the formation of a hard chancre, regional scleradenitis, which was confirmed by positive results of dark-field microscopy and increasing serological test titers.
