Angiogenesis features in patients with melanoma with different BRAf status

Abstract

Neoangiogenesis is an important factor in the development and progression of cancer. Neoangiogenesis in tumors is necessary for its further growth and subsequent metastasis. Intensity ofangiogenesis depends on a balance ofproangiogenic and antiangiogenic factors. One of these angiogenic factors is matrix metalloproteinase-9 (MMP-9) enzyme family endopeptidases participating in the extracellular matrix degradation and remodeling of vessels. It is known that the BRAF V600E mutation can affect the expression ofpro-angiogenic factors in tumor tissue. The purpose of this study was to investigate the microvasculature density in the tumor andperitumoral area in patients with melanoma with different BRAF-status using counting CD-31 positive stained vascular endothelial cells, as well as detection of the expression of matrix metalloprotieinase-9 in tumor cells and cells of microenvironment, followed by analysis of the relationships among these indicators. The material for the study is based on samples of tumor (n = 57) obtainedfrom patients with melanoma. The study revealed that there was a trend to increased angiogenesis in 2-fold (p < 0.05) with the BRAF-positive lentigo melanoma, compared with BRAF-negative patients lentigo melanoma. Vascularization level change was detected, depending on the primary tumor site: the level of vascularization was statistically higher in BRAF-positive patients with localized tumors on the skin of the trunk (p < 0.05), with no significant differences for other important morphologic parameters such as ulceration of the tumor, the severity of lymphocytic infiltration and tumor thickness by Breslow (p > 0.05). No statistically significant differences in the expression of MMP-9 depending on the BRAF-status in the tumor cells and stromal cells in microenvironment (p > 0.05) were detected. Nevertheless, a tendency to an increase in the expression of MMP-9 in the surrounding stroma cells (fibroblasts, lymphocytes, endothelial cells, polymorph nuclear leukocytes, regardless of BRAF-status) was shown. Despite some features of tumor angiogenesis in the skin melanoma patients with different BRAF status angiogenesis in the tumor is influenced by a variety of proangiogenic and antiangiogenic stimulation that are common patterns regardless of BRAF-status.

The evolution of views on the erythroderma

Abstract

Erythroderma is inflammation ofpart or all of the skin. The diagnosis of an erythroderma puts the doctor in very difficult situation as he should solve a problem with many unknowns. The commonly accepted classification of an erythroderma doesn’t exist, and classification by Louis An Jeanne Brock based on separation of erythroderma on primary and secondary, is seldom used in recent years. At the end of the XX century most of authors refused division of erythroderma on primary and secondary. It is considered that “primary” erythroderma is a toxiderma and chronically proceeding malignant lymphoma of skin. Eryhtroderma represent one of the most complex problems of dermatology. First of all, it is explained by variety of the reasons, a pathogenesis ambiguity, and rather monotonous clinical picture, tendency of many forms of an erythroderma to a long and heavy current and resistance to therapy. Molecular immunology, immunophenotyping and genotyping methods help to differentiate various forms of the disease. Evolution of views of erythroderma with the detailed description of historical aspects, modern views and convenient classification are presented.

General cryotherapy with the use of cryocapsule ICEQUEEN in patients with various dermatoses

Abstract

The article describes the modern approach to the treatment of chronic immune-mediated skin diseases using a new method of physiotherapy - general cryotherapy in cryocapsule ICEQUEEN. Combination standard treatment with drug-free and some non-drug therapies significantly improves the therapeutic effect that demonstrated in particular by the efficiency of cryotherapy. Talking about the efficiency of cryotherapy in many dermatoses authors refer not only to the literature data but also on their own experience. Pronounced clinical effect was noted in patients with psoriasis, atopic dermatitis, lichen planus, pruritus and toxiderma. Low efficiency was observed in the treatment of acne.

Expression of protein components desmosomal apparatus and cytoskeleton of the epidermis in disease Hailey-Hailey

Abstract

Hailey-Hailey disease (benign familial chronic pemphigus Guzhero-Hailey-Hailey) - hereditary vesiculo-bullous dermatosis, where mutation of ATP2C1 gene results in morphological changes, which cause an abnormal location of molecules in intercellular adhesion. It is believed that the pathology may involve both the inner surface of the plasma membrane proteins of keratinocytes and desmosomal and adhesion molecules in epidermal intercellular connections. This present research provides the results of immunohistochemical study on the distribution of desmosomal cadherin (desmogleins 1, 2, 3), desmosomal proteins (plakoglobin, desmoplakin I, periplakin), intercellular junctions molecules (E-cadherin, cadherin complex) in desmosomal apparatus and cytoskeletal proteins (cytokeratins 5, 14, suprabasal layers of epidermis) in clinically intact and affected skin of patients with Hailey-Hailey disease. The research also detected violation in localization of the protein components in the system of intercellular junctions and the cytoskeleton from severe immunohistochemical reaction to disappearance and appearance in places not typical for its localization. Thus, the most severe expression of protein molecules was observed in the areas of acantholysis and the formation of intra-epidermal blisters with the penetration of specific material into the cytoplasm of keratinocytes and some acantholytic cells. Besides, in the clinically intact skin areas of patients the most intense damage (almost complete absence of expression or its “masking”) was identified as a damage with the cadherin adhesion molecules. The paradoxical phenomenon - intranuclear and/or perinuclear luminescence was marked by cytokeratin 5, desmogleins and cadherins. The complex mechanism of Hailey-Hailey disease involving various adhesion molecules and protein components of intercellular connections system requires ongoing detailed study of the expression of multiple adhesion molecules associated with the system of intercellular junctions. The acquired knowledge will bring understanding of the sequence of events in pathology (acantholysis) development and the identification of a number of additional differential diagnostic signs of Hailey-Hailey disease.

Mistakes of diagnostics in pemphigus vulgaris: case report

The causes for the fault diagnosis of pemphigus vulgaris are discussed. The lowering of the age of patients noted the last years, clinical peculiarities similar with those in other bullous dermatoses and toxic epidermal necrolysis and multiforme exudative erythema are considered. A case in a 35-year old man who was sequentially in different clinics diagnosed Exacerbation of chronic nasopharyngitis. aphthous stomatitis, Lacunar tonsillitis. oral thrush, Chemical burn of the mucosa of the oral cavity, Chemical burn of the esophagus. Erosive - ulcerative form of lichen ruber planus of the mucosa of the oral cavity, Multiforme exudative erythema is presented. According to the diagnosis different medicines were prescribed, any improvement was not noted The vulgar pemphigus was diagnosed in 2 months after the appearance of the first eruption.

Features of patogenesis, clinical presentation and treatment of actinic ceratosis

Abstract

Actinic keratosis is a precancerous condition, which occurs as a result ofprolonged or frequent exposure to UV radiation on the skin. Treatment of actinic keratosis is effective prevention of malignancy process. Photodynamic is the most modern method of treatment of actinic keratosis. 40 patients with actinic keratosis were examined and treated by the photodynamic method using 5-aminolevulinic acid (“Alasens”) and therapeutic “Biospec” flash lamp. The high effectiveness of this method is confirmed by the results of histology, acoustic study of the skin and indicators of antioxidant status.

Case of a congenital dystrophic violent epidermolysis

Abstract

Clinical case of generalized form of congenital dystrophic violent epidermolysis at the patient M., 26 age is presented. The disease constantly progressed and proceeded with the classical picture expressed by all implications and symptoms characteristic of this form of illness. Despite of treatment new organs were involved in the process: kidneys, eyes, the hemopoietic system, developed a cachexia, contractures. Quality of life considerably worsened, difficulties in movements appeared. This case shows disease severity, gravity of the implications and complications leading to disability.

Clinical case scleroderma associated with Borrelia infection

Abstract

Clinical case of skleroderma, associated with borrelia infection is presented. 64 year old patient complained about lesions on the skin of the trunk, in the form of consolidation with change of colour and shine. 5 years ago on a skin in the left calcaneal area stuck tick that has been independently removed, was found. First occurrences of dark stains on a trunk was noticed a year ago. On the basis of the anamnesis, clinical picture and histologic research the diagnosis of localized skleroderma was established. Successful treatment was performed.

Lipidotransport system in patients with acne conglobata forms

Abstract

The lipidotransport system at conglobate acne as more severe form that characterized by pronounced hyperplasic and hypertrophic processes in sebaceous gland was studied. The study was based on the presence of relative hyperandrogenism in the pathogenesis of acne, due to the active synthesis of 5.alpha.-dihydrotestosterone sebocytes of sebaceous hair follicles of type III, and the fact that the predecessor of 5a-dihydrotestosterone is cholesterol. The study included 130 persons (60 women, 70 men) at the age of 17-36 years with acute stage of conglobate acne and disease duration from 3 months to 18 years. In addition 55 persons with papulopustule acne (23 women, 32 men) in similar age as control group were studied. A total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides were determined by semi-automatic biochemical analyzer BTS-350 using the kits of the company Deacon (Moscow). An abdominal organs and retroperitoneal space (liver, gall bladder, and pancreas) were examined by ultrasound diagnostic. Lipid metabolism changes were revealed in patients with conglobate acne, and was not found in patients with papulopustule acne. This lipid changes were associated with a tendency to increase in triglycerides, total cholesterol, lowering LDL cholesterol in HDL cholesterol content. It’s noted that in papulopustule form were not registered a higher LDL cholesterol levels but in conglobate form this parameter were 7.69% patient. With increasing duration of the disease the concentration of triglycerides, total cholesterol, LDL cholesterol is rising, while lowering levels of HDL cholesterol at the development of chronic pancreatitis.

Pyogenic granulema in dermatologist practice

Abstract

Cases of pyogenic granulema on hand and face of child which have arisen after small traumas are presented. Patients complained about tumor formations easily bleeding at an insignificant trauma. In one case histologic examination confirmed the diagnosis. Electrocoagulation of the tumor was performed. No complications were detected. Disease recurrence was observed. Dermatologists should carry out differential diagnostics with tumor formations (nodular melanoma, sarcoma of Kaposhi and others).

Retrospective analysis of the most common sexually transmitted diseases in patients in a metropolis

Abstract

Objective: to evaluate the incidence of sexually transmitted infections (STIs), in patients of State Research Center of dermatology and cosmetology Russian Ministry of Health in comparison with indicators of morbidity in the Russian Federation. Material and methods. A retrospective analysis of the structure appeals to the department of infectious urogenital diseases of Russian Ministry of Health for the period from 2011 to 2015. Results. The result of the department of sexually transmitted infections of State Scientific Center for Dermatovenereology and Cosmetologymost frequently recorded patients with anogenital (venereal) warts, syphilis and chlamydial infection. Moreover, the majority of women (52%) identified inflammatory diseases of the vagina, vulva, cervix and changes cervical mucosa. It was found that in Moscow and among patients of State Scientific Center for Dermatovenereology and Cosmetology regardless of sex the most common diseases are anogenital (venereal) warts, while in Russia and Central Federal District -urogenitalny trichomoniasis. Significant prevalence of HPV infection, in view of oncological vigilance and frequent combination with inflammatory diseases of the genital organs, demonstrates the need for prevention of infection among the active population and the fast development of effective methods of early diagnosis and therapy.
