TO THE QUESTION OF DIFFERENTIAL DIAGNOSIS OF T CELL SKIN LYMPHOMAS

Abstract

Cutaneous T cell lymphomas (CTCL) are a clinically and morphologically heterogeneous group of cutaneous malignant tumors, caused by monoclonal proliferation of lymphoid tissue cells in the skin. They are responsible for about 80% of all primary cutaneous lymphomas, while cutaneous lymphomas are responsible for 2% of all dermatological diseases. The incidence of CTCL is now increasing all over the world. For this reason, CTCL has to be diagnosed on the early stage to improve course of the disease. Mycosis fungoides, the most frequent CTCL variant, is usually diagnosed basing on clinical, histological, immunohistochemical and molecular findings. However, it can imitate other chronic dermatoses, which makes it difficult to diagnose. The article presents a review of the current literature data on new diagnostic markers of cutaneous T-cell lymphomas.

SCLEROATROPHIC LICHEN - CURRENT DATA ON THE ETIOLOGY, PATHOGENESIS, CLINICAL FEATURES AND TREATMENT

Abstract

Scleroatrophic lichen (SL) - chronic autoimmune dermatosis, is one of the clinical variants of limited scleroderma, characterized by multifactorial genesis of development against the background of immunological changes, vascular and metabolic disorders in the connective tissue, which is characterized by severe focal atrophy of the skin and mucous membranes of the external genitals. SL is a debilitating disease causing itching, pain, dysuria and impaired urination, dyspareunia and significant sexual dysfunction in women and men. This article presents a review of current literature data on the etiology and pathogenesis, clinical picture, classification, differential diagnosis of the disease, as well as methods of treatment of patients. This review is based on the analysis of 64 articles, which were obtained from eLibrary, PubMed, NCBI, Embase.

GOUGEROT-HAILEY-HAILEY’S FAMILIAL BENIGN CHRONIC PEMPHIGUS: A CASE REPORT

Abstract

The review of the literature on epidemiology, pathogenesis, diagnosis and treatment methods for chronic familial benign pemphigus Gougerot-Haley-Haley, as well as a clinical case of a patient with this disease with family history are presented. The manifestation of the disease occurred at the age of 24, after childbirth. Later there were numerous relapses. Remission was quickly achieved after treatment with antibiotics and oxygen-ozone therapy. In recent years, there has been an increase in the frequency of exacerbations of the disease, and a decrease in treatment effectiveness.

FURUNCULOID MYIASIS, DIFFICULTIES IN DIAGNOSIS

Abstract

Dermatozoonoses are skin diseases caused by arthropod pathogens. This group of skin diseases creates a particular diagnostic problem. Dermatozoonoses include deep and superficial skin changes. Superficial skin changes occur at the sites of bites or after contact with poisonous animals. Deep skin changes occur due to the penetration of the parasite into the skin. A prime example of this disease is dermatobiasis. It is an obligate myiasis, caused by the larvae of Dermatobia hominis flies (Oestridae family) that inhabit mostly in forests of Central and South America. A clinical case of the development of furunculoid myiasis in a russian geologist on a business trip to Argentina is presented.

MODERN UNDERSTANDING OF THE STRUCTURE OF SKIN MICROBIOTA IN VARIOUS DERMATOSES

Abstract

The skin ecosystem (microbiome) is a complex environment represented by a diverse community of microorganisms with various mechanisms of interaction with each other and the epidermis. The structure of colonization varies depending on the localization on the skin and environmental changes. There is a complex mechanism of the indirect influence of microorganisms through Toll-like receptors (TLR2) in the skin on the activation of the cascade of reactions leading to the production of antimicrobial peptides. This is the basis for the development of chronic inflammation, provoking mechanisms of carcinogenesis in skin cells. The mechanisms of influence of the commensal flora on the skin condition, the change in its regenerative capabilities have been little studied. Modern ideas about the importance of skin microbiota in the mechanisms of development of frequently occurring dermatoses are presented.

THE USE OF INTRAVENOUS LASER BLOOD IRRADIATION IN THE TREATMENT OF PATIENTS WITH BEHCET’S SYNDROME

Abstract

The results of a microbiological and PCR survey of patients with Behcet’s disease, as well as the results of intravenous laser irradiation of the blood (ILIB) are presented. According to the authors, the clinical picture of the disease was represented by the presence of single and multiple repeatedly recurring erosive and ulcerative defects in the area of the oral mucosa (cheek area and lateral surface of the tongue) and/or external genitalia. 4 patients noted soreness in the area of rashes, in 2 rashes were accompanied by a rise in temperature and the symptoms of malaise, weakness. Signs of catarrhal conjunctivitis and uveitis were present in 3 patients, while there was no reduction in visual acuity in the course of the disease. 3 patients had pain in large joints without visible changes on the radiograph and an increase in ESR up to 18-24 mm/h. As a result of the complex therapy with antibacterial, antiprotozoal, antiviral, antifungal therapy depending on the detected pathogen, as well as procedures of ILIB, the resorption of the infiltration zone was noted on the 10-11th day of treatment. Ulcerative elements began to epithelize. Erythema, feelings of burning and pain were absent.

ЕRYTHEMATOTELANGIECTATIC ROSACEA: THE OPTIMIZATION OF DIAGNOSTICS AND THERAPY

Abstract

Presents the modern classification, definition and differential diagnostic criteria of the clinical picture of rosacea. The social significance of dermatosis, the importance and the ability to eliminate the symptoms of the disease in the early stages, achieve a sustainable clinical effect to the ”Clear“ skin condition. The article presents the first domestic positive experience in the complex treatment of the erythematotelangiectatic rosacea, using broadband pulsed light radiation using a 0.5% brimonidine tartrate gel (Mirvazo Derm®). The effectiveness of the method is estimated using a multispectral analysis of the skin, which allows real-time to see the morphological structure of the pathological process, as well as create a map of the distribution of hemoglobin and measure its concentration in the lesions. During research there were 60 patients aged 26 to 55 years with an established diagnosis of erythematotelangiectatic rosacea. The duration of the disease ranged from 6 months to 12 years. We have demonstrated the high efficiency and tolerability of the combined method, the absence of side and undesirable effects in patients with rosacea with an erythematotelangiectatic subtype.

ASSESSMENT OF THE EFFECTIVENESS OF MICROPLASMA PIXEL RF TECHNOLOGY OF THE ACCENT PRIME MULTI-FUNCTION DEVICE FOR RADIO FREQUENCY AND ULTRASOUND THERAPY

Abstract

The article is devoted to the study of the effectiveness of the latest Pixel RF microplasma nozzle for the Accent Prime device (Alma Lasers, Israel), which allows to use cold plasma for the correction of skin pathology with minimal patient rehabilitation time, quick tissue regeneration and maximum procedure efficiency. Innovative roller nozzles with many spicule electrodes provide local penetration of plasma radio-frequency charges with the formation of microchannels in the skin with a zone of thermal plasma damage. We observed 12 women aged 30 to 40 years with manifestations of involutional skin changes in the form of wrinkles. The procedures were carried out according to the method of processing each skin crease 1 time in 2 weeks 3-4 procedures per course with research before treatment and 6 weeks after the end of the course. The procedures were performed with an individual selection of energy for each patient in the range of 10-12 watts. Studies after a course of procedures with the Microplasma Pixel RF nozzle demonstrated an improvement in the skin microrelief pattern, namely, a decrease in wrinkling by 27% from the initial level and a relief reduction by 15.67%.
