EXPERIENCE OF USING ORAL MELATONIN IN PATIENTS WITH ACNE AND CONCOMITANT SLEEP QUALITY DISORDERS

Abstract
BACKGROUND: Somnological disorders accompany many chronic dermatological diseases, affect the quality of life and the psycho-emotional state of patients, leading to a more severe course of dermatosis. According to statistics, there is a predominance of female patients who have impaired sleep quality, which is associated with hormonal fluctuations in different age periods.
AIMS: To evaluate the effect of oral melatonin intake on the severity of acne and quality of life in women.
MATERIALS AND METHODS: A prospective single-center study was conducted in the period from 2023 to 2024, 130 women with a diagnosis of mild to moderate acne were monitored, with an average age of 23,32±2,75 years. The severity of acne was calculated based on the Global Acne Assessment System (GAGS); the level of somnological disorders was determined using the Pittsburgh Sleep Quality Index (PSQI); socio-psychological testing included the determination of the dermatological Quality of Life Quality of Life Index (DLQI). Laboratory control included determination of LH, FSH, 17-OHP, DHAE-S, free testosterone, IGF-1, insulin and glucose levels with calculation of HOMA-IR insulin resistance index.
RESULTS: In patients with acne, signs of sleep disorders were reported in 65,38% of cases (n=85), the severity of which correlated with the severity of dermatosis (p=0,565; p<0,001). Sleep disorders in patients with acne revealed a more pronounced decrease in quality of life (p<0,001) and a significant increase in free testosterone (p<0,001) and IGF-1 (p=0,008). Complex therapy using means for the external treatment of acne and melatonin contributed to a faster regression of clinical manifestations of dermatosis, improved indicators of socio-psychological indices and sleep quality.
CONCLUSIONS: Sleep disorders have a high incidence in women with mild to moderate acne. Timely detection of signs of sleep disorders and their correction increases the clinical effectiveness of therapy with normalization of patients' quality of life.
