Immune-inflammatory rheumatic diseases, such as rheumatoid arthritis (RA), significantly increase CVR. <bold>Purpose</bold>: In an open comparative observational study, to analyze traditional and disease-associated cardiovascular risk factors in patients with RA and arterial hypertension (AH), and to determine the level of circulating markers of systemic inflammation and endothelial dysfunction. <bold>Materials and methods</bold>: The study included 150 patients with established RA (ACR/EULAR criteria, 2010) and 50 individuals with hypertension. In addition to the standard examination stipulated by current clinical guidelines for RA and hypertension, the presence and severity of joint syndrome, the presence and type of concomitant cardiovascular pathology were taken into account in patients with RA. Laboratory tests included acute-phase reactants, immunological markers of RA, and specific laboratory parameters: quantitative levels of tumor necrosis factor-α, interleukin-1β, interleukin-6, endothelin-1, and von Willebrand factor in the patients' blood. Statistical processing of the results was performed using parametric analysis methods with normal data distribution. <bold>Results and discussion:</bold> Patients in the study (RA) and control (AG) groups did not differ significantly in demographic and anthropometric parameters. More than half of the patients with RA (62.67 %) fell into the very high or high CV risk category. A comparison of the frequency of traditional risk factors in the study groups revealed that patients with RA had higher systolic blood pressure, higher mean total cholesterol levels, and less frequent use of antihypertensive therapy. Disease-associated cardiovascular risk factors were present in the vast majority of patients with RA and were more frequently recorded in individuals with high and moderate RA activity. Atherosclerotic CVD was represented by various forms of IHD, and CHF was present in every fifth patient with RA. Patients with RA had a significant increase in the levels of soluble markers of endothelial dysfunction – endothelin-1 and von Willebrand factor, as well as a high level of circulating pro-inflammatory cytokines – TNF-alpha, IL-1, and IL-6. <bold>Conclusions</bold>: Thus, the vast majority of patients with RA have comorbid cardiovascular disease, which has demonstrated a direct association with RA disease activity. A significant increase in key proinflammatory cytokines, combined with elevated levels of endothelin-1 and von Willebrand factor, was detected in the blood of patients with RA, confirming the role of systemic inflammation in the development of vascular lesions in RA.</p></abstract><trans-abstract xml:lang="ru"><p>Сердечно-сосудистые заболевания (ССЗ) остаются ведущей причиной смертности и утраты трудоспособности. Иммуно-воспалительные ревматические заболевания, такие как ревматоидный артрит (РА), существенно повышают сердечно-сосудистый риск (ССР). <bold>Цель.</bold> В открытом сравнительном наблюдательном исследовании провести анализ традиционных и болезнь-ассоциированных факторов ССР у пациентов РА и артериальной гипертензией (АГ), а также определить уровень циркулирующих маркеров системного воспаления и дисфункции эндотелия. <bold>Материалы и методы. </bold>В исследование включено 150 больных с достоверным РА (критерии ACR/EULAR,2010) и 50 человек с АГ. Наряду с общепринятым обследованием, предусмотренным действующими клиническими рекомендациями по РА и АГ, у пациентов с РА учитывали наличие и выраженность суставного синдрома, наличие и форму сопутствующей кардиоваскулярной патологии. Спектр лабораторных тестов включал острофазовые показатели воспаления, иммунологические маркеры РА, а также специальные лабораторные показатели: количественное содержание фактора некроза опухоли-α, уровня интерлейкина-1β, интерлейкина-6, эндотелина-1 и фактора фон Виллебранда в крови пациентов. Статистическую обработку результатов проводили с помощью методов вариационного анализа при нормальном распределении данных. <bold>Результаты и обсуждение.</bold> Пациенты основной (РА) и контрольной групп (АГ) существенно не различались по демографическим и антропометрическим показателям. Более половины пациентов с РА (62,67 %) соответствовали категории очень высокого и высокого ССР. Сравнение частоты регистрации традиционных ФР в наблюдаемых группах показало, что у пациентов с РА был выше показатель систолического АД, средний уровень общего холестерина, реже применялась антигипертензивная терапия. Болезнь-ассоциированные факторы кардиоваскулярного риска имели место у подавляющего большинства пациентов с РА и чаще регистрировались у лиц с высокой и умеренной активностью РА. Нами была выявлена прямая ассоциация между степенью активности РА и частотой регистрации ССЗ. У пациентов с РА обнаружено достоверное повышение уровня растворимых маркеров дисфункции эндотелия – эндотелина-1 и фактора Виллебранда, а также высокий уровень циркулирующих провоспалительных цитокинов – ФНО-альфа, ИЛ-1, ИЛ-6. <bold>Заключение. </bold>Таким образом, у подавляющего большинства пациентов с РА имеется сопутствующая кардиоваскулярная патология, которая продемонстрировала прямую ассоциацию с активностью РА.
