Phototherapy modalities, including psoralen plus ultraviolet A (PUVA) and photodynamic therapy (PDT), are widely used as skin-directed therapies for early-stage MF. However, comparative data on their efficacy remain limited.</p> <p><bold>AIM:</bold> To conduct a comparative retrospective analysis of the efficacy of PUVA therapy and PDT in patients with MF.</p> <p><bold>METHODS: </bold>The study included 90 patients with a verified diagnosis of mycosis fungoides: 52 patients received PUVA therapy (4 times weekly; oral oxsoralen or ammifurin; initial UVA dose 0.5 J/cm² with gradual increase) and 38 patients received PDT (once weekly; topical spirulina gel containing chlorin e6; wavelength 66±2 nm; light dose 200–400 J/cm²). The following outcomes were assessed: rates of complete and partial regression of skin lesions, remission duration, number of sessions, and the influence of age, sex, and lesion type on treatment outcomes.</p> <p><bold>RESULTS:</bold> In the PUVA group, complete response (CR) was achieved in 71.2% of patients (37/52), and partial response (PR) in 28.8% (15/52). Median remission duration was 24 months (95% CI: 18–30). A strong correlation was found between the number of sessions and remission duration (ρ = 0.70; p &lt; 0.001). ROC analysis identified a threshold value of ≥25 sessions (sensitivity 83.8%, specificity 66.7%). In the PDT group, no complete responses were observed; partial response was achieved in 68.4% of patients (26/38), and no response was observed in 31.6% (12/38). Median remission duration was 12 months (95% CI: 11–13). According to Cox regression, PDT was associated with a 2.18-fold increased risk of relapse compared to PUVA (p = 0.004). The presence of nodular lesions was an unfavorable prognostic factor (hazard ratio = 1.95; p = 0.018). Age and sex did not influence treatment efficacy (p &gt; 0.05).</p> <p><bold>CONCLUSION: </bold>PUVA therapy is a highly effective treatment for mycosis fungoides, achieving complete regression in 71.2% of patients with a median remission duration of 24 months. PDT using the studied protocol demonstrated significantly lower efficacy (no complete responses, median remission 12 months). A minimum of 25 PUVA sessions is recommended to achieve complete regression</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Грибовидный микоз (ГМ) — наиболее частая форма Т-клеточной лимфомы кожи. Фототерапевтические методы, включая ПУВА-терапию и фотодинамическую терапию (ФДТ), широко применяются при ранних стадиях заболевания, однако сравнительных исследований их эффективности недостаточно.</p> <p><bold>Цель исследования. </bold>Провести сравнительный ретроспективный анализ эффективности ПУВА-терапии и ФДТ у пациентов с ГМ.</p> <p><bold>Методы. </bold>В исследование включено 90 пациентов с верифицированным диагнозом грибовидного микоза: 52 пациента получали ПУВА-терапию (4 раза в неделю, оксаролен или аммифурин внутрь, начальная доза УФА 0,5 Дж/см² с постепенным увеличением) и 38 пациентов — ФДТ (1 раз в неделю, гель спируллины с хлорином Е6, длина волны 66±2 нм, доза 200–400 Дж/см²). Оценивались частота полного и частичного регресса высыпаний, длительность ремиссии, количество сеансов, а также влияние возраста, пола и типа высыпаний на исходы.<bold> </bold></p> <p><bold>Результаты. </bold>В группе пациентов, получавших ПУВА полный регресс был достигнут у 71,2% пациентов (37 из 52), частичный — у 28,8% (15 из 52). Медиана длительности ремиссии составила 24 месяца (95% ДИ: 18–30). Выявлена сильная корреляция между количеством сеансов и длительностью ремиссии (ρ = 0,70; p &lt;0,001). ROC-анализ определил пороговое значение ≥25 сеансов (чувствительность 83,8%, специфичность 66,7%). В группе ФДТ полный регресс отсутствовал, частичный регресс отмечен у 68,4% пациентов (26 из 38), отсутствие эффекта наблюдалось — у 31,6% (12 из 38). Медиана длительности ремиссии составила 12 месяцев (95% ДИ: 11–13). По данным регрессии Кокса, ФДТ ассоциировалась с 2,18-кратным увеличением риска рецидива по сравнению с ПУВА (p = 0,004). Наличие узловых форм высыпаний являлось неблагоприятным прогностическим фактором (отношение рисков = 1,95; p = 0,018). Возраст и пол не влияли на эффективность терапии (p&gt; 0,05).<bold> </bold></p> <p><bold>Заключение. </bold>ПУВА-терапия является высокоэффективным методом лечения грибовидного микоза, обеспечивая полный регресс у 71,2% пациентов и медиану длительности ремиссии 24 месяца. ФДТ по использованному протоколу демонстрирует значительно более низкую эффективность (отсутствие полных регрессов, медиана ремиссии 12 месяцев).
