Despite the high prevalence of the condition, no systematic data exist in the Russian population on the frequency of micronutrient deficiencies, hormonal disorders, or their relationship to the clinical manifestations of the disease.</p> <p><bold>AIM:</bold> This study aimed to assess the prevalence of ferritin and vitamin D deficiency and other laboratory abnormalities in women with childbearing potential with acute telogen effluvium, and to determine the relationship between identified disorders and trigger factors as well as phototrichogram indicators.</p> <p><bold>METHODS:</bold> From September to December 2025, an observational clinical study with analytical components was conducted at specialized clinical diagnostic centers in Moscow and the Department of Skin and Venereal Diseases named after V.A. Rakhmanov. Included were women aged 18–45 years with clinical signs of acute diffuse telogen effluvium, disease duration of 2–6 months, and a preserved menstrual cycle. Excluded were patients with decompensated endocrine, oncological, and autoimmune diseases, as well as pregnant and breastfeeding women. Clinical data analysis, phototrichogram with determination of the proportion of telogen hairs, and comprehensive laboratory assessment of iron metabolism (ferritin, transferrin saturation coefficient), vitamins, trace elements, and hormonal profile (thyroid status, androgens) were performed.</p> <p><bold>RESULTS:</bold> The study examined 60 patients. Mean age was 28.4±4.2 years, and mean hair loss duration was 3.8±1.3 months. Ferritin deficiency (&lt; 30 ng/mL) was detected in 34 (56.7%), and vitamin D deficiency (&lt; 30 ng/mL) in 41 (68.3%). The proportion of telogen hairs was 23.4±4.8% in the parietal area and 19.7±3.9% in the occipital area (normal value: up to 15%). The most common trigger factors were previous infections (63.3%), psychoemotional stress (56.7%), and weight changes (28.3%). A correlation was found between ferritin level and the proportion of telogen hairs (<italic>p</italic>=0.041). The mean Dermatology Life Quality Index score was 8.6±4.2, indicating a moderate impact on quality of life.</p> <p><bold>CONCLUSION:</bold> Patients with acute telogen effluvium had a high prevalence of ferritin and vitamin D deficiency, justifying the need for mandatory laboratory screening of these parameters. Study limitations included the small sample size and the absence of a control group.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Острое телогеновое выпадение волос остаётся недостаточно изученным в аспекте комплексной оценки триггерных факторов и лабораторных маркеров у женщин репродуктивного возраста. Несмотря на высокую распространённость проблемы, в российской популяции отсутствуют систематизированные данные о частоте дефицита микронутриентов, гормональных нарушений и их взаимосвязи с клиническими проявлениями заболевания.</p> <p><bold>Цель исследования</bold> ― оценить распространённость дефицита ферритина, витамина D и других лабораторных отклонений у женщин репродуктивного возраста с острым телогеновым выпадением волос, а также определить связь выявленных нарушений с триггерными факторами и показателями фототрихограммы.</p> <p><bold>Методы.</bold> В наблюдательное клиническое исследование с элементами анализа на базе специализированных клинико-диагностических центров г. Москвы, кафедры кожных и венерических болезней имени В.А. Рахманова в период с сентября по декабрь 2025 года включались женщины 18–45 лет с клиническими признаками острого диффузного телогенового выпадения волос длительностью заболевания 2–6 месяцев и сохранённым менструальным циклом. Исключались пациентки с декомпенсированными эндокринными, онкологическими, аутоиммунными заболеваниями, беременные и женщины в период грудного вскармливания. Проводился анализ клинических данных, фототрихограммы с определением доли телогеновых волос, комплексного лабораторного исследования с оценкой обмена железа (ферритин, коэффициент насыщения трансферрина), витаминов, микроэлементов и гормонального профиля (тиреоидный статус, андрогены).</p> <p><bold>Результаты.</bold> Обследованы 60 пациенток, средний возраст 28,4±4,2 года, длительность выпадения волос 3,8±1,3 месяца. Дефицит ферритина (&lt; 30 нг/мл) выявлен у 34 (56,7%), недостаточность витамина D (&lt; 30 нг/мл) ― у 41 (68,3%). Доля телогеновых волос составила 23,4±4,8% и 19,7±3,9% в теменной и затылочной зонах (норма до 15%). Наиболее частыми триггерными факторами были перенесённые инфекции (63,3%), психоэмоциональный стресс (56,7%) и изменения массы тела (28,3%). Выявлена корреляция между уровнем ферритина и долей телогеновых волос (<italic>р</italic>=0,041). Средний балл по дерматологическому индексу качества жизни составил 8,6±4,2, что соответствует умеренному влиянию на качество жизни.</p> <p><bold>Заключение.</bold> У пациенток с острым телогеновым выпадением волос обнаружена высокая распространённость дефицита ферритина и витамина D, что обосновывает необходимость обязательного лабораторного скрининга этих показателей.
