Conventional therapies, such as minoxidil and 5α-reductase inhibitors, offer limited efficacy and are associated with adverse effects, highlighting the need for novel, pathogenetically grounded approaches. Recently, exosome-based therapeutics – particularly those derived from non-human sources – have emerged as a promising avenue in regenerative trichology.</p>  <p> </p><p><bold>Aim.</bold> To evaluate the clinical efficacy and pathogenetic rationale of salmon-derived exosomes in patients with androgenetic alopecia based on phototrichogram data and expression of cellular proliferation (Ki-67) and angiogenesis (CD31+) markers in scalp skin biopsies.</p>  <p> </p><p><bold>Materials and Methods</bold><bold>.</bold> A prospective, single-group interventional study was conducted involving 20 patients (14 males with AGA stages III–IV on the Norwood–Hamilton scale and 6 females with stages I–II on the Ludwig scale). All participants underwent 4 sessions of intradermal injections of salmon-derived exosomes, followed by microneedling, at 14-day intervals. Efficacy was assessed at week 12 using phototrichogram analysis (ARAMO SG, TrichoScan v4.5) and histological/immunohistochemical evaluation of scalp biopsies (markers: Ki-67, CD31).</p>  <p> </p><p><bold>Results.</bold> By week 12, all key parameters showed significant improvement: hair density increased by +44.4 hairs/cm² (p&lt;0.001), the proportion of vellus hairs decreased from 40.4% to 3.4% (p=0.005), mean hair shaft diameter increased by 22.4 µm (p&lt;0.001), and the proportion of anagen hairs rose by +36.4% (p&lt;0.001). Histological analysis confirmed an increase in follicular density (+1.4 follicles/mm², p&lt;0.001) and normalization of the terminal/vellus ratio (from 2.3 to 9.6, p&lt;0.001). Immunohistochemistry demonstrated enhanced cellular proliferation (Ki-67 in the outer root sheath: +55.5%, p&lt;0.001) and angiogenesis (CD31+: +11.5 vessels per field of view, p&lt;0.001). No serious adverse events were recorded.</p>  <p> </p><p><bold>Conclusion.</bold> Monotherapy with salmon-derived exosomes combined with microneedling represents a highly effective and safe treatment for AGA, exerting pathogenetically justified effects by restoring the functional reserve of hair follicles, reversing miniaturization, and normalizing the follicular microenvironment. These results position salmon-derived exosomes as a promising cell-free regenerative strategy for clinical practice.</p>  <p> </p><p><bold>Keywords.</bold> Androgenetic alopecia, exosomes, salmon-derived exosomes, phototrichogram, Ki-67, CD31.</p> </abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Андрогенетическая алопеция (АГА) является наиболее распространённой формой нерубцовой алопеции, имеющей генетическую предрасположенность и полигенный характер наследования. Заболевание характеризуется прогрессирующей миниатюризацией терминальных волос в андрогензависимых областях под влиянием дигидротестостерона, что приводит к их замене на веллусные волосы и заметному снижению качества жизни пациентов. Препаратами первой линии терапии АГА являются миноксидил и ингибиторы 5-α-редуктазы. Однако в последнее время отмечается увеличение числа случаев недостаточного ответа на стандартную терапию, что обусловливает поиск инновационных патогенетически обоснованных подходов, основанных на принципах регенеративной медицины, в частности экзосом.</p> <p><bold>Цель исследования.</bold> Оценить клиническую эффективность и патогенетическую обоснованность применения экзосом из тканей лосося у пациентов с андрогенной алопецией на основании данных фототрихограммы, экспрессии маркеров клеточной пролиферации (Ki-67) и ангиогенеза (CD31+) в биоптатах кожи волосистой части головы.</p> <p><bold>Материалы и методы.</bold> Проведено проспективное одногрупповое интервенционное исследование с участием 20 пациентов (14 мужчин с АГА III–IV стадий по Норвуду-Гамильтону и 6 женщин с I-II стадиями по Людвигу). Все пациенты получили 4 сеанса введения лиофилизированных экзосом из тканей лосося с последующим микронидлингом с интервалом в 14 дней. Оценка эффективности терапии проводилась до лечения и на 12-й неделе после последней процедуры на основании фототрихограммы (ARAMO SG, TrichoScan v4.5), гистологического и иммуногистохимического анализа биоптатов кожи волосистой части головы (Ki-67, CD31).</p> <p><bold>Результаты.</bold> На фоне проводимой терапии зарегистрировано статистически значимое увеличение плотности волос на +44,4 волоса/см² (<italic>p</italic>&lt;0,001), понижение доли пушковых волос с 40,4 % до 3,4 % (<italic>p</italic>=0,005), увеличение среднего диаметра стержня волос на 22,4 мкм (<italic>p</italic>&lt;0,001), доли анагеновых волос на +36,4% (<italic>p</italic>&lt;0,001). Гистологический метод показал увеличение плотности терминальных волосяных фолликулов (+1,4 фолл./мм², <italic>p</italic> &lt; 0,001), а также нормализацию T/v-соотношения (соотношение терминальных/веллусных волос) с 2,3 до 9,6, <italic>p</italic> &lt; 0,001. Иммуногистохимическое исследование продемонстрировало достоверное увеличение пролиферации Ki-67 в наружной корневой оболочке волоса (+55,5 %, <italic>p</italic>&lt;0,001) и CD31+ (маркер ангиогенеза; +11,5 сосудов/поле зрения, <italic>p</italic>&lt;0,001). Серьёзных побочных эффектов выявлено не было.</p> <p><bold>Заключение.</bold> Применение экзосом, полученных из тканей лосося, в сочетании с микронидлингом представляет собой высокоэффективный и безопасный метод терапии АГА. Полученные результаты подтверждаются восстановлением функционального резерва волосяных фолликулов, значительным снижением миниатюризации волос, нормализацией микроокружения (включая улучшение ангиогенеза и снижение воспаления).
