The etiological and pathogenetic mechanisms underlying this condition are insufficiently studied; however, they are critical for distinguishing between livedoid vasculopathy and similar skin diseases and developing effective therapeutic strategies. Clarifying the pathogenesis and developing precise diagnostic criteria will improve differential diagnosis, accelerate its verification, and thereby promote quality of life through combination therapy.</p> <p>The paper presents a clinical case in a 56-year-old female patient with long-term livedoid vasculopathy secondary to insulin-independent diabetes mellitus and genetically determined thrombophilia caused by numerous mutations in hemostasis genes. The diagnostic workup included a differential diagnosis with ulcerative necrotic polymorphic dermal angiitis and chronic varicose ulcers. A comprehensive examination confirmed the final diagnosis of livedoid vasculopathy.</p> <p>This case highlights the critical role of extended diagnostic workup in patients with chronic skin ulcers, which is essential for a timely, accurate diagnosis and pathogenetically appropriate therapy, with a special focus on concomitant thrombophilia.</p></abstract><trans-abstract xml:lang="ru"><p>Диагностика и лечение ливедоидной васкулопатии остаются одними из ключевых и наиболее сложных задач современной дерматовенерологии. Несмотря на недостаточную изученность этиологических и патогенетических механизмов данного заболевания, именно их выявление является основой для определения нозологической принадлежности ливедоидной васкулопатии среди сходных дерматологических заболеваний и разработки эффективных терапевтических стратегий. Уточнение патогенеза и разработка чётких диагностических критериев позволят улучшить дифференциальную диагностику, сократить сроки верификации заболевания и, как следствие, повысить качество жизни пациентов благодаря применению комбинированных подходов к терапии.</p> <p>В статье приводится клиническое наблюдение 56-летней пациентки с длительным течением ливедоидной васкулопатии на фоне инсулиннезависимого сахарного диабета и генетически детерминированной тромбофилии, обусловленной множественными мутациями в генах, задействованных в системе гемостаза. В ходе диагностического поиска осуществлена дифференциальная диагностика процесса с язвенно-некротической формой полиморфного дермального ангиита и хроническими варикозными язвами. Комплексное обследование позволило верифицировать окончательный диагноз ливедоидной васкулопатии.</p> <p>Представленный случай иллюстрирует важность расширенного диагностического поиска у пациентов с хроническими язвенными поражениями кожи, что является необходимым условием для своевременной постановки точного диагноза и назначения патогенетически обоснованной терапии, при этом особое внимание при ведении подобных пациентов уделяется сопутствующей тромбофилии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>livedo</kwd><kwd>livedoid vasculopathy</kwd><kwd>livedo vasculitis</kwd><kwd>argon plasma</kwd><kwd>thrombophilia</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>ливедо</kwd><kwd>ливедоидная васкулопатия</kwd><kwd>ливедо-васкулит</kwd><kwd>аргоновая плазма</kwd><kwd>тромбофилия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Borzova EYu, Vertieva EYu, Grabovskaya OV, et al.
