A control group comprised 20 conditionally healthy volunteers. The severity and dynamics of clinical symptoms during treatment were assessed using the Hidradenitis Suppurativa Severity Index. T helper cells (Th) were analyzed using CD3 and CD4 antibodies. Th cell differentiation was assessed with CD45RA and CD62L markers. Serum interleukin levels were measured via enzyme immunoassay. All 12 patients with hidradenitis suppurativa received secukinumab for 16 weeks.</p> <p><bold>RESULTS:</bold> Positive changes in the clinical course of hidradenitis suppurativa were observed within one week of initiating secukinumab gene-engineered therapy, with continued regression of efflorescences leading to significant clinical improvement by the end of the 16-week treatment course. Treatment resulted in normalization of peripheral blood levels of Th17 and Treg cells, as well as Th17/Treg and IL-17A/IL-10 ratios.</p> <p><bold>CONCLUSION:</bold> A pronounced Th17/Treg imbalance may serve as a marker of disease severity in hidradenitis suppurativa and a rationale for initiating biologic therapy with secukinumab.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Гнойный гидраденит является относительно редким дерматозом с тяжёлым хроническим течением, крайне резистентным к рутинным методам лечения, что значительно снижает качество жизни пациентов и представляет значимую медико-социальную проблему.</p> <p><bold>Цель исследования</bold> ― клинико-иммунологическая оценка эффективности секукинумаба у больных гнойным гидраденитом.</p> <p><bold>Методы.</bold><bold> </bold>В исследовании приняли участие 12 пациентов (7 лиц мужского и 5 лиц женского пола) в возрасте от 12 до 52 лет с диагнозом гнойного гидраденита. Контрольная группа состояла из 20 условно здоровых добровольцев. Для оценки тяжести течения гнойного гидраденита и динамики изменений клинической симптоматики заболевания в процессе лечения использовали индекс тяжести гнойного гидраденита (HSSI) по балльной системе. Т-хелперы (Тh) исследовали с помощью антител против CD3 и CD4. Для изучения процессов дифференцировки Т-хелперов применяли антитела к CD45RA и CD62L. Содержание интерлейкинов в сыворотке крови определяли методом иммуноферментного анализа. У 12 пациентов с гнойным гидраденитом курс лечения секукинумабом составил 16 недель.</p> <p><bold>Результаты.</bold> В процессе применения генно-инженерной терапии положительные сдвиги в течении гнойного гидраденита стали прослеживаться спустя неделю от начала лечения секукинумабом, что позволило вследствие дальнейшего регресса эффлоресценций добиться значительного клинического улучшения к окончанию 16-недельного курса. На фоне терапии секукинумабом наблюдалась нормализация в периферической крови относительного содержания Тh17, Treg, соотношений Th17/Treg и IL-17А/IL-10.</p> <p><bold>Заключение.</bold> Значительный дисбаланс в соотношении Th17/Treg можно расценивать как один из маркеров тяжести течения гнойного гидраденита и основанием для назначения генно-инженерной биологической терапии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>hidradenitis suppurativa</kwd><kwd>mononuclear cells</kwd><kwd>interleukins</kwd><kwd>secukinumab</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>гнойный гидраденит</kwd><kwd>мононуклеарные клетки</kwd><kwd>интерлейкины</kwd><kwd>секукинумаб</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Burova SA, Potekaev NN, Borodulina KS.
