Moreover, Human class II leukocyte antigen (HLA) alleles are known to be the best-established predisposing factor for its development. Class II HLA alleles vary among racial/ethnic backgrounds.</p> <p><bold><italic>AIM:</italic></bold> we investigated an association between HLA class II DRB1 alleles and pemphigus vulgaris among the Russian population.</p> <p><bold><italic>MATERIALS AND METHODS:</italic></bold> A total of 70 patients with confirmed diagnosis of pemphigus vulgaris were genotyped for HLA class II alleles. HLA class II frequencies were compared with healthy volunteers (<italic>n</italic>=92). The statistical significance of differences between patients and controls was evaluated using Fisher’s exact test. A single blood sample for HLA typing was obtained from all the subjects. The strength of association between HLA alleles and pemphigus vulgaris was estimated by odds ratio (OR) and 95% confidence intervals (CIs) <italic>p</italic> &lt;0.01 was considered to be statistically significant. P-values were corrected for multiple comparisons according to the Benjamini–Hochberg method.</p> <p><bold><italic>RESULTS:</italic></bold> We observed that DRB1*4 and DRB1*14 allelic groups were associated with pemphigus vulgaris (41.4% vs 8.7%; OR 2.33 vs 0.34; <italic>p</italic>=0.000001 and 27.1% vs 6.5%; OR 1.98 vs 0.41; <italic>p</italic>=0.0004 respectively), while HLA-DRB1*11 and HLA-DRB*15 were negatively associated with pemphigus vulgaris (4.3% vs 21.7%; OR 0.33 vs 1.62; <italic>p</italic>=0.001 and 5.7% vs 19.6%; OR 0.44 vs 1.5 respectively).</p> <p><bold><italic>CONCLUSIONS:</italic></bold> Our findings suggest, that HLA DRB1*4 and DRB1*14 alleles are genetic markers for pemphigus vulgaris susceptibility, whereas DRB1*11 and HLA-DRB*15 groups appear protective in Russian population.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование.</italic></bold> HLA аллели (лейкоцитарные антигены человека) II класса ― одни из ключевых пусковых механизмов развития истинной акантолитической пузырчатки. Существует большое количество исследований, демонстрирующих, что различные HLA аллели являются предрасполагающими к развитию истинной акантолитической пузырчатки, тогда как другие играют протективную роль. Кроме того, частота встречаемости HLA-DRB1 аллелей в российской популяции не исследовалась.</p> <p><bold><italic>Цель исследования</italic></bold> ― оценить частоту HLA-DRB1 аллелей у больных истинной акантолитической пузырчаткой в российской популяции, а также выявить предрасполагающие и протективные HLA аллели.</p> <p><italic>Материал и методы.</italic> В исследовании случай-контроль, выполненном в период с 2016 по 2021 г., приняли участие 70 больных истинной акантолитической пузырчаткой и 92 здоровых донора, подписавших информированное согласие. У всех испытуемых был получен один образец крови для HLA-типирования. Первичная конечная точка исследования ― сравнительный анализ частоты HLA-DRB1 аллелей у больных истинной акантолитической пузырчаткой и здоровых доноров в российской популяции с помощью метода Qiagen QIAamp DNAMini Kit для HLA-типирования.</p> <p><bold><italic>Результаты.</italic></bold> DRB1*4 (41,4 против 8,7%; ОШ 2,33 и 0,34; <italic>p</italic>=0,000001) и DRB1*14 аллели (27,1 против 6,5%; ОШ 1,98 и 0,41; <italic>p</italic>=0,0004) статистически значимо чаще встречались у пациентов с истинной акантолитической пузырчаткой по сравнению с группой контроля соответственно, тогда как DRB1*11 (21,7 против 4,3%; ОШ 1,62 и 0,33; <italic>p</italic>=0,001) и DRB1*15 (19,6 против 5,7%; ОШ 1,5 и 0,44; <italic>p</italic>=0,01) ― достоверно чаще у здоровых доноров по сравнению с больными истинной акантолитической пузырчаткой.</p> <p><bold><italic>Заключение.</italic></bold> В проводимом исследовании удалось выявить аллели, предрасполагающие к развитию истинной акантолитической пузырчатки, а именно HLA-DRB1*4 и DRB1*14, в то время как HLA-DRB1*11 и DRB*15 являлись протективными аллелями, т.е. встречались у больных истинной акантолитической пузырчаткой статистически значимо реже по сравнению с группой здоровых доноров.</p></trans-abstract><kwd-group xml:lang="en"><kwd>pemphigus vulgaris</kwd><kwd>HLA</kwd><kwd>DRB1</kwd><kwd>protective alleles</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>истинная акантолитическая пузырчатка</kwd><kwd>HLA аллели</kwd><kwd>DRB1 аллели</kwd><kwd>предрасполагающие HLA аллели</kwd><kwd>протективные HLA аллели</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Petzl-Erler ML.
